Financial Policy

In order to keep costs down, payment is expected at the time of your visit. Payment can be made by check, travelers checks, or credit card. There is a monthly $10.00 collection charge for all over due bills.

A SuperBill receipt will be provided and may be used for insurance reimbursement. Finance and courtesy plans are available for those in need. All arrangements must be made at the time of the initial visit and payment schedules must be made on time or the special arrangements are cancelled automatically.

If bills are sent, 30 days are allowed without a service charge. Overdue accounts are charged a monthly service charge of 1.5% or $7.00, whichever is greater. A $45.00 fee will be charged for all NSF checks.

Insurance Policy

Our receipt may be used instead of filling out the Attending Physician's portion of the insurance form. All medical insurance companies accept this method of reimbursement. Insurance reimbursement will be made directly to you unless other arrangements are made in advance. Please check with your health insurance company to determine their policy. We accept Blue Cross and Blue Shield PPO. Please check with your insurance carrier before making an appointment. Medicare patients must pay their annual deductible, and will then receive a courtesy discount as determined by Medicare guidelines. We will compete Medicare forms for your convenience and Medicare will reimburse you directly.

Private Pay Patients

If you do not have Blue Cross or Blue Shield Insurance, you are considered a private pay patient. Several patients from across the US have visited our office. We try and make this experience as convenient as possible. Any patient who visits our office can then be treated, if they are followed by their family physician.

You can fly into San Jose (one hour away) or directly to Monterey airport (10 minutes), or take the airport shuttle from SFO or SJC to the Alvarado station in Monterey, which is 6 minutes from my office, by foot. The site for the airbus is: http://www.montereyairbus.com/

We also have made arrangements with the Quality Inn at 1058 Munras Avenue to accommodate our visiting patients. It is located across the street from our office and the telephone number is

Fees for initial consultation are $749 for out of state and $599 for California residents. This fee does not include laboratory testing which can run from $100 to $399 depending on how extensive the testing. We use Esoterix Laboratories for our hormone studies.
